You will need to provide the following information:

1. Patient’s date of birth
2. Patient's Name
3. Home address or current location of the patient
4. Telephone number for the triage nurse to call back
5. Name of patient’s Own GP
6. Patient's Medical Card or GP Visit card details if applicable, or method of payment.
7. Brief summary of the patient’s symptoms

A triage nurse will call you back to assess your medical needs

You will be asked to provide the following patient information:

• Symptoms
• Characteristics
• Past medical history
• Onset of symptoms
• Location of symptoms
• Aggravating factors
• Relieving factors
• Allergy Information
• Current Medications

This information will determine the appropriate course of action for the patient

The appropriate care may include one of the following possible outcomes

• Nurse Advice
• Doctor Advice
• Treatment Centre Appointment
• Home Visit
• Emergency Services

Details of the SouthDoc consultation will be sent directly to the patient’s GP the following morning.
